Home at Last is a vital new community service. If you are an older adult and are being discharged from hospital alone either from an impatient stay or from an emergency visit ask your nurse or doctor about Home At Last. This smooth transition is achieved by coordinating an enhanced hospital discharge process to meet a pre-determined discharge time, with community support services arranging transportation home and a Personal Support Worker to assist the patient to get settled. The worker may prepare a homecoming meal, pick-up prescriptions and basic groceries, and remain with the patient for up to six hours. The next day, the Home at Last Care Coordinator will follow up with the patient and family to arrange any needed community services and supports so the patient will get well at home and reduce the possibility of readmissions to the hospital.
